How do I correct an error in my Social Security Number, change my beneficiary or change my (or my Authorized Signer's) last name due to marriage or divorce?
Answer:
Corrections to a Social Security Number must be submitted to us with a W9 form. A W9 can be obtained from www.irs.gov. Name changes for you or your authorized signer can be made by submitting the Account Information Change Form. This form can be found within the Accountholder tab, under Online Forms.
